When you fill out our contact form on our contact page we ask that you provide the following case details:

The information collected from you helps inform our free initial consultation process, which is usually conducted in our office. In some instances, we may ask you to fill out an additional online form called “Free Consultation Form.” If you consent to filling out this form, then we ask that you provide personal information.
